About HCM5003: Concepts and Leadership Theories in Health and Care Assignment
The HCM5003 assignment is designed to help students show how leadership thinking connects with the daily realities of health and care practice. It is not about repeating textbook definitions, but about choosing one real issue in a service and linking it to leadership ideas that can guide change. For example, you might look at a team with low morale and explain how a transformational approach could build confidence, or how a situational style could help a manager adapt support for different staff members.
Strong answers are short, focused, and rooted in practice. Instead of listing many models, compare two or three and show how they would work in your chosen setting. Use plain examples such as handover delays, staff turnover, or patient complaints, then show how leadership theory can address these. Finish with a brief reflection on what the theory taught you about improving care and managing people.
Key Points for Concepts and Leadership Theories in Health and Care Assignment:
Choose one service issue and stay focused.
Apply leadership theory to action , not just description.
Show clear outcomes for staff, patients, or the organisation.
Reflect on your learning and personal leadership growth.

Executive Summary
This portfolio has been developed for Greenvale Community Health Trust , a mid-sized care provider rated “Requires Improvement” by the Care Quality Commission (CQC). The report highlights leadership and cultural issues identified during inspection, particularly inconsistent safety reporting and unclear governance structures. By applying leadership theories such as transformational, servant, and distributed leadership, the consultancy provides a route for improvement that builds trust, strengthens accountability, and promotes openness in practice. Strategic tools, including PESTEL and SWOT, are also introduced to support recruitment, mission alignment, and long-term planning. The recommendations are designed to guide senior leaders in shifting from compliance-driven behaviour to proactive quality improvement, with the overall aim of achieving a “Good” rating and creating a safer, more engaged environment for both staff and patients.
Introduction to the Portfolio
The purpose of this portfolio is to support improvement in an organisation that has been rated “Requires Improvement” by the Care Quality Commission (CQC). The chosen case is Greenvale Community Health Trust , which provides community nursing, therapy, and rehabilitation services. Its inspection report raised two critical issues: firstly, an uneven safety culture where incidents were not always reported or investigated properly; and secondly, a weak governance structure that left staff uncertain about decision-making and accountability. These weaknesses not only place patients at risk but also reduce staff confidence in leadership. This portfolio begins by outlining the concepts of organisational culture and leadership theories within health and care. It then presents Greenvale’s profile and challenges, followed by a detailed report on recruitment, mission, and organisational tools, before offering leadership development advice to the board.
Part 1: Introduction and Analysis
LO1. Examine the role and importance of traditional and more recent leadership theories and concepts for the health and care setting.
In health and care, culture can be understood as “the way things are done every day.” It includes the beliefs, behaviours, and routines that guide how staff treat patients, how teams interact, and how problems are solved. A strong culture values openness, teamwork, and learning from mistakes. In services rated “Requires Improvement,” cultural gaps often appear as poor communication, fear of speaking up, or inconsistent standards between departments.
Leadership is the main driver of culture. In a hospital or community service, leaders set the tone by how they respond to problems, communicate expectations, and involve staff. Good leaders balance the need for compliance with the need for compassion and motivation. In practice, leadership is not just about managers – it also comes from ward leads, senior nurses, therapists, and other frontline figures.
Theories Relevant to Improvement
Several leadership theories can be applied to support Greenvale’s improvement:
Transformational leadership : Leaders inspire staff by sharing a clear vision and encouraging innovation. In practice, this means explaining why change is needed, not just what must be done.
Transactional leadership : Focuses on structure and accountability. For Greenvale, this might mean standardising incident reporting and monitoring follow-up actions.
Servant leadership : Puts staff needs first. This helps rebuild trust when staff feel unsupported, encouraging them to raise concerns safely.
