Assessment 2: Case Study - Description

Person-centred care can be described as ‘patient-centred care’ and ‘client-centred care’ in which the focus is ‘on the needs of the person rather than the needs of the service’ (Royal College of Nursing, 2016). The Health Foundation (2015, p.3) advise that person-centred care requires a collaborative approach in coordinating and tailoring care to individual needs, whilst providing “knowledge, skills and confidence [needed] to…make informed decisions about their own health and health care.”

With reference to the statements above and [Mental Health/Adult] Nursing, you will be required to complete a 3000-word case study to demonstrate the knowledges, skills, qualities and behaviours toward achieving the module learning outcomes

 

Scenario to base your case study on

Tom is 58 years old. He has Early onset dementia, heart failure and renal cancer. He is in hospital for treatment for ulcers on his leg. He lives at home with his wife Hannah who is 56years old and still works as a school teacher. The couple live in their own home but have a son who is dependent on them as he is Autistic and needs regular support. They have daily care at home and Tom is very resistive to any compromise of his independence. Tom is at the end of his life and you have an opportunity to explore his situation and consider what aspect of his needs you would like to address and provide an evidence based approach to how you might care for him and Hannah.

The general purpose of the case study is to:

  • Identify an individual situation (case), Tom and Hannah`s scenario
  • identify the key issues of the case – issues you can explore , you need to prioritise 2/3 actionable needs
  • analyse the case using relevant theoretical concepts from your learning about nursing in a person centred way
  • recommend a course of action for that particular issue using evidence

  • Biopsychosocial perspectives and impacts on his illness’s- dependent son and
  • Communication skills and development of therapeutic relationship, including
